Pharmacy Team

Clinical pharmacists work as part of the general practice team to improve value and outcomes from medicines and consult with and treat patients directly. 

Paramedics

Our team of paramedics work across all member practices, providing face to face appointments in the practice as well as visiting housebound patients in their homes.

Social Prescribers

Social prescribing connects people to non-medical support to address these issues and other unmet needs.

Health And Wellbeing Coach

Our Health and Wellbeing Coach support our Social Prescribing team in connecting patients to non-medical support to address their issues and other unmet needs.

Care Coordinator

Our Care Coordinator works with our Social Prescribing and Wellbeing teams to support our patients in unmet needs.
